Best Practice Case Study – Wealdon District Council
The Health Coach Project in Herstmonceux, East Sussex, is a pioneering example of social prescribing. The project started in April 2018, following a pilot which began in February 2017, after GP John Simmons saw a need to treat patients more holistically. A partnership between the Herstmonceux surgery, Freedom Leisure and Wealden District Council was set up. In the summer of 2019, it expanded to two further GP surgeries in Hailsham. It is funded by the NHS Eastbourne, Hailsham and Seaford Clinical Commissioning Group.
